Instructions:

1. Include your resume showing work history (if any), accomplishments, community involvement, awards, etc.

2. Write a 250 - 500 word essay explaining your reason for pursuing a career in healthcare, your passion for helping others, and any particular life experiences that has shaped who your are.

3. Obtain two letters of recommendation from teachers, employers, or healthcare professionals.

​

4. Have your high school, college, or trade school email your official transcript to: info@mujangsfoundation.org (Subject: Scholarship)

If you do not meet the minimum required criteria, please include a letter of explanation. You may still be considered for a scholarship. Mujang's Foundation understands that everyone has unique life circumstances.
